Teams are required to submit a completed Team Roster and, where applicable, Guest Player Forms prior to the tournament.

Guest Player Form
All players not registered on the team’s regular 2025/26 indoor season roster must be submitted with a completed Guest Player Form along with their roster.
Guest players must be listed on the team roster.
District approval is required if the guest player is registered outside your district.
